Become A Unitedhealthcare Provider

With more than 645,000 doctors and 5,105 hospitals, UnitedHealthcare is one of the largest health care provider networks in the U.S. UHC has a presence in all 50 states, and when you, the provider, join the network, UHC members can easily find you in a provider search. They can then visit your office to receive in-network insurance benefits. UHC features many online services to its network providers, including claims submission and the ability to check member eligibility, view payment statements and receive notification of updates to policies, procedures and protocols.


Instructions


1. Call UnitedHealthcare's provider services department at 877-842-3210 to request an application to join the network. UHC will send you the application with instructions on where to submit it.


2. Complete the hard copy application. If you prefer to apply online, visit UHC's provider portal (see Resources). Select "New User" and enter your tax identification number to access the site.


3. Provide UHC with access to your credentialing information through the Council for Affordable Quality Healthcare (CAQH). If you have not registered with CAQH, visit its website to sign up at no cost (see Resources). Several other insurance carriers use CAQH as the single source of credentialing information, so you will need to complete this information only once for multiple network applications.


4. Submit the application and supporting documentation according to the instructions on the application. UHC will notify you if it is missing any information necessary for its review.


5. Contact UHC at 877-842-3210 to check the status of your network application if you have not heard back within 30 to 45 days of initial submission. UHC will send you a written notification of your acceptance or denial into its network. Sign and send back any related contracts or agreements and other required supporting documentation.
