Challenge The Nclex Exam

The National Council Licensure Examination (NCLEX) is an exam given by the National Council of State Boards of Nursing to license nurses in the United States. NCSBN offers a formal exam review or challenge for candidates that failed the NCLEX exam to dispute the validity of the failed items. You can challenge the entire exam or individual exam items by completing the NCLEX Review and Challenge Form and paying the required fee.


Instructions


1. Contact the NCSBN's examinations department at 866-293-9600. Request a NCLEX Review and Challenge Form. NCSBN will mail or fax the form to you.


2. Complete the NCLEX Review and Challenge Form. You can choose to have the entire exam reviewed or you can choose to have individual items on the exam reviewed.


3. Return the NCLEX Review and Challenge Form to: National Council of State Boards of Nursing Inc., Examinations Department, 111 E. Wacker, Suite 2900, Chicago, Illinois, 60601 with a money order or certified check for the appropriate fees. As of January 2011, the NCSBN charges $400 for a full exam review or $90 plus $110 for each individual item challenged.
